Enrollment Requirements

Please prepare the following documents based on your enrollment type.

New Students
  • ✓LRN (Learner's Reference Number)
  • ✓Report Card / Form 138
  • ✓Good Moral Certificate
  • ✓Form 137 / SF 10
  • ✓PSA Birth Certificate (Original)
Kinder
  • ✓PSA Birth Certificate (Original)
ESC / PEAC — Incoming Grade 7 (FREE Tuition)
  • ✓2x2 ID Pictures
  • ✓PSA Birth Certificate
  • ✓Proof of Income — ITR (if employed, photocopy)
  • ✓Affidavit of Non-Filing of Income Tax (if unemployed)
  • ✓Certificate of Employment (if employed, original)
  • ✓Grade 6 Report Card (photocopy)
